VNA launches Medicare dementia support program
VNA of the Treasure Coast has been approved as a provider for the Guiding an Improved Dementia Experience Model, a Medicare program that supports people with suspected or confirmed dementia and the family members who care for them.
The program, known as GUIDE, is designed to improve quality of life for people living with dementia and reduce strain on family caregivers through care coordination, education and Medicare-covered respite services.
More than 11 million Americans provide unpaid care for someone living with dementia, often managing appointments, coordinating care, assisting with daily activities and offering emotional support. VNA said the GUIDE Model gives families additional resources to help loved ones remain safely at home for as long as possible.
“Family caregivers are the backbone of care for many older adults living with dementia,” said Carmen Luca, director of private care at VNA of the Treasure Coast. “The GUIDE Program provides caregivers with education, support, resources and respite so they can navigate this journey with greater confidence and less stress. By supporting caregivers, we are also helping people with dementia remain safely and independently in their homes for as long as possible.”
Eligible participants receive support from a dedicated dementia care navigator, who serves as a primary point of contact for individuals and families. The navigator helps coordinate care and connect families with community resources, including transportation services, meal programs, safety programs and other local support services.
The program also offers caregiver education and support to help families better understand dementia, manage stress, improve safety and build confidence in their caregiving role. Participants may qualify for up to $2,500 per year in Medicare-covered in-home respite care, allowing caregivers to take a temporary break while their loved one continues receiving care at home.
